Your Favorite Hot Cocoa Recipe?

I’ve been trying to find a hot recipe that tastes really good (no, not just good), to give as a “homemade holiday gift”, but have been very unsuccessful. I have looked on cdkitchen, Food Network, and just plain googling, but have not found a recipe that does not leave a bitter after taste or one that leaves you wondering “where is the chocolate?” watery sensation.
Can you please list your favorite recipes as a comment to better my search for the best Hot Cocoa Mix.

I like the recipe right off of the Hersheys Baking Cocoa container:
for 6 servings
1/2 c. sugar
1/4 c. Hersheys Cocoa
dash salt
1/3 c hot water
4 c. milk
3/4 tsp. vanilla extract
Stove:Mix sugar, cocoa and salt in sauce pan; stir in water. Cook and stir over medium heat until mixture boils; boil and stir for 2 minutes. Stir in milk and heat. DO NOT BOIL. Remove from heat; add vanilla.
(I don’t add the vanilla. Single serving can be done in microwave also, by following almost the same directions, heating for 1 1/2 – 2 minutes)
Or for Single servings:
2 Tbsp. Sugar
3 tsp. Hersheys Cocoa
dash salt
1 c. milk
1/4 c. vanilla
